Qualifications

• Active Occupational Therapist license in the state you work.
• One year of experience as an Occupational Therapist delivering patient care.

Responsibilities

• Deliver skilled, patient-centered home health care in collaboration with an interdisciplinary team.
• Perform comprehensive ADL and upper-extremity evaluations and develop individualized treatment plans.
• Educate patients and caregivers on adaptive techniques, assistive devices, energy conservation, and home safety.
• Ensure thorough, compliant EMR documentation.
• Travel to patient homes within the assigned territory.
