Job summary

Elite Home Health Group INC in Port St. Lucie, FL seeks a compassionate Occupational Therapist to provide skilled therapy in patients' homes, assessing function, planning care, and coordinating with the interdisciplinary team.

You will evaluate, treat, educate patients and families, document care per Medicare and state rules, supervise OTA when needed, and travel within the territory to support independence and safe home-based living.

Qualifications

• Graduate of an accredited Occupational Therapy program and an active OT license.
• Minimum of one year of occupational therapy experience; home health preferred.
• CPR certification and valid driver’s license required.

Responsibilities

• Perform patient evaluations and develop individualized OT treatment plans.
• Provide skilled therapy in patients' homes per physician orders and plan of care.
• Educate patients and caregivers on safety, independence, and daily living tasks.
• Document progress, outcomes, and plan of care with compliance to regulations.
• Supervise OTA and Home Health Aide per policy and needs of care plan.
