Job summary

FOX Rehabilitation is expanding in Mantua, NJ, seeking licensed Physical Therapists to deliver one-on-one geriatric care in homes and senior living communities.

You’ll manage your own schedule, treat in private residences or senior living settings, and may incorporate telehealth to extend access. Join a clinician-led practice focused on independence, evidence-based care, and meaningful patient outcomes.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions supporting wellness, mobility, and independence.
• Document effectively within FOX’s EMR system per regulations.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate.
