Job summary

FOX Rehabilitation is expanding in Budd Lake, NJ and seeks licensed Physical Therapists to deliver one-on-one care to older adults in private residences or senior living communities, using their Geriatric House Calls model.

You’ll evaluate, create plans, and provide evidence-based therapy, with autonomy to manage your schedule and collaborate with an interdisciplinary team. Some telehealth may be incorporated for flexibility and continuity of care.

Qualifications

• Valid Physical Therapy license in the state of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support patient wellness, longevity, mobility, and functional independence.
• Document effectively in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety, function, and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
