Job summary

Acloché LLC is seeking a dedicated Physical Therapist to join our home health team in the Denver/Lancaster area. This full-time role focuses on delivering patient-centered therapy in patients' homes, with evaluation, planning, and ongoing education as core duties.

Qualified candidates have a Master’s in Physical Therapy, a current state license, CPR certification, and at least nine months of PT experience; a driver’s license is required for travel to clients.

Qualifications

• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Must possess and maintain CPR certification.

Responsibilities

• Provides patient care and documents treatment per physician orders, agency policy, and patient needs.
• Performs patient evaluations and selects diagnostic material to assess care.
• Informs physician of assessment results and recommended interventions prior to initiating care.
• Develops and documents treatment plans with appropriate interventions and priorities.
• Delivers organized patient treatment including advising, educating, and counseling the patient.
• Presents patient information to physicians and keeps the team informed of status and plan.
• Provides treatment within agency and payer parameters.
• Sets objective, measurable goals in collaboration with team and patients.
• Meets agency productivity standards and manages time effectively.
