Job summary

Endeavor Health seeks an Advanced Practice Provider (APN/PA) for Post-Acute Care in Evanston, IL. The role involves autonomous patient management within skilled nursing facilities and collaboration with physicians to optimize outcomes.

Hours are full-time (avg 40 hours/week) with a daytime schedule, no evenings or holidays initially, and potential weekend hours later. IL licensing and DEA are required, with prior APP experience preferred.

Qualifications

• License: Advanced Practice Licensure in IL OR PA licensure; DEA and IL Controlled Substance License required.
• Master’s level education in specialty area is required.
• BLS/CPR certification required.
• Prior APP experience preferred.

Responsibilities

• Provide care to patients under direction of collaborating physicians.
• Perform history, physical exams, orders and interprets imaging/tests, and writes progress notes.
• Prescribe pharmacologic and non-pharmacologic therapies as appropriate.
• Engage in learning needs assessment for patients and families and promote wellness.
• Collaborate with interdisciplinary team to ensure safe transitions of care.
