Job summary

Moments Hospice is seeking an Admissions RN for a full-time, on-road role in Minnesota. The position offers a company car with fuel and insurance covered, and a compensation package designed to support work-life balance.

You will join a team focused on compassionate end-of-life care and admissions coordination. The role emphasizes autonomy, relationship-building with referral sources, and educational outreach to patients and families while ensuring clinical competence and adherence to admission

Qualifications

Valid RN license in the state where duties are performed.
1-2 years nursing experience preferred.
Driver's license and vehicle required.
Utmost compassion and integrity.
Strong presentation skills.
Previous hospital, physician practice, LTC or ALF experience preferred.

Responsibilities

Participate in information gathering, analysis, education and form completion aspects of the admissions process.
Evaluate patients for hospice appropriateness, e.g. medical records review as requested.
Obtain agreement for admission of referred patients and families who meet admission criteria.
Analyze data collected to facilitate/obtain admission orders.
Build effective referral source relationships to support Moments Hospice quality and responsiveness.
Maintain clinical and professional competence.
Take the initiative in developing relationships internally with admissions and patient care staff.
Educate patient and family about hospice and palliative care.
