Job summary

Trellis Services is hiring a Case Manager to program manage cases and provide Family Training for households receiving Autism Waiver Services. You will oversee supervising instructors, develop and implement treatment plans, and monitor progress, ensuring timely reporting and crisis intervention when needed.

This role emphasizes working in home-based settings and coordinating with families, instructors, and Trellis staff to deliver consistent, confidential client services.

Qualifications

• Certified Special Educator or Master's in Special Education with 5 years experience with Autism.
• Ability to work in a home environment independently.
• Proof of current auto insurance, valid driver’s license and reliable transportation.
• Pre-employment criminal background check is required.

Responsibilities

• Provides Family Consultation to families and/or caregivers served under the Autism Waiver program.
• Supervises and trains Instructors assigned to individual families.
• Develops and implements written Treatment Plan(s) within 30 days of start; submits to Director.
• Develops and implements goals based on Trellis Services Curriculum.
• Monitors goals with data collection; reports progress against deadlines.
• Creates monthly instructor work schedules and approves hours.
• Available in person or by direct phone contact for crisis intervention.
• Follows Procedures for Crisis Situations and Behavior Intervention Plan (BIP).
• Holds at least one monthly collaboration meeting with instructors and families.
• Maintains consistent communications with staff, families and Trellis administration.
• Accurately record time sheets and input into Trellis database by deadlines.
• Fosters positive relationships with Trellis, staff, clients and communities.
• Maintains Confidential Clinical File with session notes, timesheets, calendars and reports.

Education

Master's Degree in Special Education or related field
