Creed Medical Group Inc. is seeking a Family Consultant and Case Manager to coordinate Autism Waiver participant services and support families. You will combine family consultation, case management, treatment planning, staff supervision, and quality assurance to deliver person-centered services.

Requirements include a Bachelor’s degree in a related field, 2 years of relevant experience, CPR/First Aid, a valid driver’s license, and full-time availability. Pay is $25 per hour.

Qualifications

• Bachelor’s degree required.
• Minimum two years’ experience with Autism Spectrum Disorder or related populations.
• Valid driver’s license and reliable transportation.
• CPR and First Aid certification required.

Responsabilités

• Coordinate services and families for Autism Waiver participants.
• Provide family consultation and education to caregivers.
• Develop and review individualized treatment plans with measurable goals.
• Supervise Direct Support Professionals and ensure compliance.
• Complete documentation and progress notes accurately and on time.
