Job summary

CenterWell Home Health seeks a Speech Language Pathologist to join its patient-focused team delivering in-home therapy across Beaumont, TX. You will evaluate, treat, and coordinate care to improve communication, swallowing and quality of life for patients in their homes.

Collaboration with clinicians and caregivers is essential to develop effective plans of care. This role emphasizes compassionate, evidence-based practice with opportunities to work within a national Humana-affiliated network,

Qualifications

• Six months of experience as a speech-language pathologist.
• Current and unrestricted license to practice.
• CPR certification current; valid driver's license and reliable transportation required.
• Home health experience preferred.

Responsibilities

• Evaluate, direct and provide speech-language pathology services to patients in the home or facility.
• Develop and periodically review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to improve function.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe, record and report changes in condition and response to treatment to supervisor/physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation and progress reports as required by policy.
• Participate in care coordination activities and discharge planning.
• Maintain patient confidentiality in all communications.
