Job summary

CenterWell Home Health in Florida’s Sebring area seeks a dedicated Home Health Physical Therapist Assistant to deliver in-home PT under the supervision of a Physical Therapist. You will implement the prescribed plan, monitor progress, and help patients regain strength, mobility and independence in the comfort of their homes.

You will document records weekly, participate in team in-services, and ensure infection control and safety standards are followed.

Qualifications

Current state license to practice Physical Therapy (PTA).
Valid driver's license, auto insurance and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of geriatric population changes.
Read, write, and speak fluent English.

Responsibilities

Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
Provide non-complex active and passive manual therapies and modalities.
Document client records and submit clinical documentation weekly.
Attend educational meetings and coordinate department in-services.
Notify PT or Branch Director of equipment failures, supply shortages, or staff changes.
Apply infection control and universal precautions in patient care.
