Job Summary

Job Summary:

Under the supervision of the Director of Training, the Associate Trainer for Health Home will play a vital role in supporting the training and development of staff within the Health Home program. This position involves designing, implementing, and evaluating training programs that enhance the skills and knowledge of team members, ensuring they are equipped to provide high-quality care and support to members. The Associate Trainer will work closely with program leadership and staff to identify training needs and develop effective training materials and sessions.

Responsibilities
  1. Training Development:
    1. Collaborate with program leadership to assess training needs and develop comprehensive training programs for Health Home staff.
    2. Assist with creating training materials (synchronous and asynchronous), including manuals, presentations, and e-learning modules, tailored to various learning styles.
  2. Training Delivery:
    1. Conduct onboarding virtual or in-person training sessions, ongoing professional workshops, and seminars for new hires and existing staff on topics related to Health Home services, evidence best practices, and compliance.
    2. Utilize various training methods, including hands-on activities, role-playing, and group discussions, to enhance learning outcomes.
  3. Evaluation and Feedback:
    1. Assess the effectiveness of training programs through participant feedback, assessments, and performance metrics.
    2. Continuously improve training content and delivery methods based on course evaluation results and emerging best practices.
  4. Support and Mentorship:
    1. Provide ongoing support and mentorship to staff, answering questions and offering guidance on Health Home policies and procedures.
    2. Foster a positive learning environment that encourages professional growth and development.
  5. Documentation and Reporting:
    1. Maintain accurate records of training sessions, participant attendance, and evaluation results.
    2. Prepare reports on training outcomes and make recommendations for future training initiatives.
  6. Collaboration:
    1. Work closely with interdisciplinary teams to ensure training aligns with program goals and client needs.
    2. Participate in team meetings and contribute to the overall success of the Health Home program.
Qualifications
  1. Bachelor's degree in social sciences (social work, psychology, public health, education, or a related field).
  2. Master's degree preferred.
  3. Previous experience in training, education, or program development, preferably in a healthcare or social service setting.
  4. Strong understanding of Health Home services, policies, and best practices.
  5. Excellent communication and presentation skills, with the ability to engage and motivate diverse audiences.
  6. Proficient in using training software and tools, as well as Microsoft Office Suite.
  7. Ability to work independently and collaboratively in a fast-paced environment.
  8. Familiarity with online training tools (LMS) is a plus.
  9. Strong written and verbal communication skills (Public speaking).
  10. Excellent customer service skills required.
  11. Effective multi-tasking and time management skills.
  12. Comfortable with conducting training in large groups, live and virtually.
  13. Must be computer savvy and have the ability to prepare presentations, and work independently.
  14. Experience with creating curriculums, training slides, presentations, and job aids.
  15. Knowledge of adult learning styles.

Working Conditions:

  1. This position may require occasional travel to various training locations.
  2. Flexibility in scheduling may be necessary to accommodate training sessions.
