Job summary

CareMore Health Management Services, LLC is seeking an Associate Practice Manager to lead day-to-day care center operations serving Medicare and Medicaid patients with chronic conditions. You will drive staffing, patient access, and patient experience while supporting value-based care outcomes.

Qualifications

  • High school diploma and a minimum of 3+ years’ experience supporting operations within a physician office, clinic, or primary care environment.
  • Bachelor’s degree may be preferred. Supervisory or team lead experience preferred.
  • Bilingual (English/Spanish) skills may be required depending on location or patient population.
  • Active Basic Life Support (BLS) certification may be required.
  • Satisfactory completion of a Tuberculosis test is required.

Job Description Summary

This role provides operational leadership for one or more CareMore Health care centers serving Medicare and Medicaid patients with chronic and complex conditions. The manager is responsible for ensuring effective day‑to‑day operations, workforce leadership, regulatory compliance, and strong patient experience while supporting value‑based care outcomes. The scope of responsibility may vary based on care center size, patient population, operational complexity, and number of locations assigned. The Associate Practice Manager oversees operations in care centers with moderate operational scope, focusing on clinical efficiency, team performance and patient experience.

Success in this role looks like care center operations run smoothly with minimal workflow disruptions. Care center teams are engaged, supported, and held accountable for their roles and responsibilities. Patient access and visit flow are effectively coordinated to support timely care delivery. Providers and care teams collaborate to deliver coordinated, patient‑centered care. Patients experience timely service and positive interactions with the care center team.

  • Operational Impact: Supports day‑to‑day operational performance of the care center by monitoring workflows, staffing alignment, and operational activities. Escalates operational issues that may impact care center performance.
  • Efficiency and Workflow Management: Monitors scheduling, patient flow, and office processes to support efficient care center operations. Identifies workflow challenges and partners with leadership to implement improvements.
  • Clinical Excellence and Execution: Supports care center operations that enable providers and care teams to deliver high‑quality care. Ensures staff follow established workflows that support patients with chronic and complex health needs.
  • Communication: Communicates operational updates, expectations, and concerns clearly with staff and providers. Builds collaborative working relationships across care center teams.
  • Leadership: Provides day‑to‑day supervision of administrative and clinical support staff. Leads coaching, accountability, and team engagement.
Minimum Qualifications

Requires a high school diploma and a minimum of 3+ years’ experience supporting operations within a physician office, clinic, or primary care environment; or any combination of education and experience which would provide an equivalent background. Bachelor’s degree may be preferred. Supervisory or team lead experience preferred. Bilingual (English/Spanish) skills may be required depending on location or patient population. Active Basic Life Support (BLS) certification may be required. Satisfactory completion of a Tuberculosis test is required.

Care center assignments and scope may evolve over time based on organizational growth, market needs, and changes in care delivery models, including the expansion, consolidation, or redesign of care center operations.

Compensation

$61,226.00 to $91,839.00
