UnitedHealth Group's Optum is hiring an Associate Patient Care Coordinator in Scottsdale, AZ. You will handle front office functions, including appointment scheduling, patient registration, cashiering, and answering calls to keep the care team connected with patients. This role supports the POP!

Team to engage patients on their health care journey. Hours are Monday–Friday, 8:00 AM–5:00 PM, at 21803 N Scottsdale Rd, Suite 100.

Qualifications

• High School Diploma or GED required.
• 1+ year of customer service experience.
• Basic proficiency with Microsoft Office applications.

Responsibilities

• Schedule appointments and register patients at the front desk.
• Answer incoming calls and perform clerical tasks.
• Assist with cashiering and MyChart enrollment.
• Support patient outreach and value-based care initiatives.
• Maintain accurate patient records and communication.

The Associate Patient Care Coordinator is responsible to the Center Administrator and provides appointment scheduling, patient registration, cashiering, answers incoming calls and performs other related clerical functions. This primary role provides front office support functions.

The purpose of the POP! Team is to actively connect with patients on their health care journey by providing information, support, and to encourage them to take proactive steps towards managing their health, ultimately aiming to improve care outcomes and adherence to treatment plans by maintaining consistent communication and appointment assistance.

Hours: Monday - Friday, 8:00AM - 5:00PM

Location:21803 N Scottsdale Rd, Suite 100 Scottsdale, AZ 8525

Primary Responsibilities:

• Outreaching to new patients to welcome them to the practice and scheduling their first two appointments with their care provider
• Identifying patients who may be overdue for preventative care or testing and working to re-engage those individuals
• Effectively communicating the importance of value-based care initiatives and how we partner with the patient to achieve better health results through preventative care
• Promote and assist with MyChart sign-ups to improve direct communication between patients and clinical staff.
• Patient Outreach:
• Work new member roster to identify patients who are new members to us, or new to Medicare; Contact patient to introduce them to Optum, and their new PCP. Have robust Value-Based Care knowledge
• Patient engagement: Encourage MyChart enrollment for streamlined communication and self-service; Provide guidance on using MyChart for appointments, refills, and direct queries.
• Re-engage patients who are overdue for preventive care and testing (Mammogram, Colorectal cancer screening, etc.)

Required Qualifications:

• High School Diploma/GED
• 1+ years of experience in customer service
• Basic level of proficiency with Microsoft Excel, Word, Outlook, and PowerPoint

Preferred Qualifications:

• Bilingual (English/Spanish)

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $16.00 to $29.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
