Job summary

Gotham Enterprises Ltd. in Oakland, CA is seeking an Associate Marriage and Family Therapist to join its Integrated Behavioral Health team as an IBH Care Coordinator.

The role focuses on coordinating care across primary care, behavioral health, housing, and social services for underserved Alameda County communities, with supervision toward licensure available. You will conduct assessments, support care plans, facilitate referrals, document in the EHR, and participate in multidisciplinary

Qualifications

• Registered Associate Clinical Social Worker (ASW/ACSW) in CA.
• Bachelor’s or Master’s degree in Social Work, Psychology, Public Health, or related field.
• Minimum 1 year of behavioral health, case management, or social services experience.
• Familiarity with Alameda County care systems (Medi‑Cal and community resources).
• Trauma-informed and culturally responsive service delivery.

Responsibilities

• Deliver in-person and telehealth care coordination for behavioral health and social needs.
• Conduct comprehensive client assessments and contribute to care plans.
• Facilitate warm handoffs and referrals to behavioral health, medical, housing, and community resources.
• Monitor engagement and support follow-up to maintain continuity of services.
• Document services accurately in the EHR and support data collection for reporting.
• Collaborate with care teams and external partners in planning.
