Job summary

Discovery Living Inc. seeks an Associate Coordinator to join a team delivering supported living services to individuals with developmental disabilities in the community. Works under the supervision of the Community Living Coordinator.

Responsibilities

  • Provide professional direct support and supervision per the member's Plan of Care.
  • Assist with personal hygiene, meal planning and preparation, and clothing care.
  • Support social skills, transportation, communication, and recreation.
  • Administer prescribed medications and/or first aid as needed.
  • Complete documentation and required reports in electronic systems.

Job Summary

The Associate Coordinator is an active member of a team providing supported living services to individuals with developmental disabilities who reside in the community. The Associate Coordinator works under the direction of the Community Living Coordinator.

Essential Functions
  1. Provide the member with professional direct support, supervision and skill training, as identified in the member’s Plan of Care (POC). Areas may include, but are not limited to:
  2. Personal Hygiene
  3. Meal Planning and Preparation
  4. Clothing Care
  5. Social Skills
  6. Transportation
  7. Communication
  8. Medication Administration
  9. Personal Safety
  10. Money Management
  11. Home Maintenance/Housekeeping
  12. Recreation
  13. Community Inclusion
  14. Communicate effectively with the member’s family.
  15. Provide informal counseling and promote self-advocacy.
  16. Assist with medical appointments.
  17. Administer prescribed medications and/or first aid.
  18. Participate in scheduled support team meetings.
  19. Participate in the member’s interdisciplinary team meeting.
  20. Complete documentation of all services and authorized supports.
  21. Complete the annual assessment with each member and update as needed through the year.
  22. Review all program records (documentation).
  23. Assist the Community Living Coordinator in meeting member needs and supported community living regulations.
  24. Attend mandatory training.
  25. Complete daily documentation
  26. Be available as 1st contact for home needs from 8:00 AM Monday through 4:30 PM Friday.
  27. Complete end of month paperwork in a timely manner to meet billing requirements.
Requirements
Qualifications

A high school diploma or GED is required. Some college education is preferred. One year of experience working with individuals who have developmental disabilities is preferred, but not required. The successful completion of training to dispense medications to members will be required. Must have a valid Iowa driver’s license and be insurable through the agency insurance carrier.

Competencies

Individuals working with members in the Supported Community Living Program must be able to:

  1. Provide support services in a way that relies on mutual respect between members and support staff.
  2. Demonstrate a dedication to member empowerment by providing decision-making opportunities in all aspects of daily living.
  3. Must be able to understand, access, and utilize company computer hardware, software, and systems in order to promptly and accurately document and submit required reports, data, and other forms of electronic documentation.
  4. Organize work, be good at solving problems, be a sound decision‑maker, communicate needed information to supervisor and other team members, and give careful attention to details.
  5. Cooperate with team members to accomplish team and organizational goals and be able to adjust to, and manage, change.
  6. In addition, the individual must be dependable and enthusiastic about performing the job to the best of their ability. They must give attention to their personal appearance and reflect a positive image in the community, and must work diligently toward member community inclusion.
  7. Because the work schedule may vary from day to day and week to week, the Associate Coordinator must be flexible and willing to work with the support team in meeting the needs of the members.
