Job summary

Marin Community Clinics is seeking an Associate Case Manager to join our Complex Care team in Marin County. You will support a caseload of 30-40 patients, develop care plans with patients, address social factors, and coordinate care with a multidisciplinary team to improve health outcomes.

The role requires MA/CHW/BS in social work or equivalent, 1–2 years’ relevant experience, Spanish language ability, and proficiency with Microsoft Office.

Qualifications

• MA, CHW, or Bachelor of Social Work or equivalent case management experience required.
• 1–2 years’ experience in supporting individuals with chronic/complex BH or medical needs preferred.
• Housing resources/navigation experience in Marin preferred.
• Knowledge of state regulations and patient confidentiality essential.
• Motivational interviewing training is desirable.

Responsibilities

• Support a minimum caseload of 30–40 Complex Care patients.
• Implement comprehensive patient care plans in partnership with patients.
• Triage and respond to tasks and care needs.
• Assist patients to complete forms and applications.
• Address social determinants of health (SDOH) and connect to resources.
• Coordinate care with PCP, behavioral health, and other team members.
• Document interactions in MCC’s EHR and secondary databases.
• Attend meetings and participate in trainings as required.

Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. Marin Community Clinics is looking for a mission-driven Associate Case Manager to join our dynamic, multidisciplinary, Complex Care team to advance our goal of improving health outcomes for individuals with complex medical, behavioral health, and psychosocial needs. Our Complex Care team is committed to increasing health equity among Marin’s most vulnerable patients through person-centered, trauma-informed, case management support. Working collaboratively with primary care, behavioral health, and other members of the clinical care team, the Associate Case Manager works as a member of an expert team of professionals to provide patient-centered, outcome- driven care that promotes independence and dignity.

Responsibilities

• Support a minimum caseload of 30-40 Complex Care patients.
• Implement comprehensive patient care plan in partnership with patients.
• Triage and respond to tasks/care needs.
• Assist Patients to complete necessary forms/applications.
• Work with Patients to address social determinants of health (SDOH).
• Assist patients in getting connected to community resources.
• Assist patients to schedule/attend medical, BH, and specialist appointments.
• Administrative support including records retrieval and review, eligibility screening, goal tracking, and more.
• Assist patient to coordinate all necessary care.
• Conduct home/community visits as needed.
• Consult/coordinate with interdisciplinary care team, including behavioral health, care navigation MAs, nursing, medical providers, and external partners.
• Document all Patient interactions and services using MCC’s electronic health record AND secondary database.
• Attend internal and external meetings (i.e. case conferencing, administrative).
• Attending trainings such as health coaching, engagement techniques, etc.
• Timely and accurate reporting of data as requested to MCC CM programs.
• May travel between clinics or meet Patients in the community as needed.
• Provide frank and considered opinions regarding case management services and quality improvement measures.
• Provide services to satellite clinics as needed; may be asked to work variable schedule during extended-hour clinics (e.g., 10am-7pm on one or more days).
• Other duties may be assigned.

Qualifications

Education and Experience:

• MA, CHW, Bachelor of SW or equivalent case management experience required.
• 1-2 years’ experience (and passion for) providing support to individuals with chronic and complex BH needs, complex medical conditions and/or substance-using individuals highly preferable.
• Experience/familiarity with housing resources/housing navigation in Marin highly preferable.
• Knowledge of state regulations and legal/ethical standards related to patient rights, and client/patient confidentiality required.
• Training in motivational interviewing highly desired/or willingness to learn.

Required Skills and Abilities:

• Must be fluent in English with the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
• Ability to write reports and business correspondence in English.
• Must be able to communicate with patients and partners in Spanish: effectively present information and respond to questions from clients, customers, and the public in Spanish.
• Must be able to conduct clinical assessment interviews and case management interventions with patients in Spanish.
• Willing to drive to other MCC Clinic locations in Marin County.

Computer Skills:

• To perform this job successfully, an individual must be computer literate and knowledgeable in Microsoft Office software programs, and how to conduct searches on the Internet.

Certificates, Licenses, Registrations:

• BLS Training Current.

Physical Requirements and Working Conditions:

• Prolonged standing and walking.
• Prolonged periods of sitting at a desk and working on a computer.
• Must be able to lift to 15 pounds at times.
• Will drive among clinic locations as needed.
• Fulfill immunization and fit for duty regulatory requirements.
