Job summary

Marin Community Clinics in Novato, CA seeks a Dental Care Navigator to advocate for patients and coordinate questions about treatment needs within the dental department.

Under the supervision of the Chief Dental Officer, you will help patients gather documents, collaborate with dental professionals, and support scheduling, outreach, and data entry across MCC clinics.

Qualifications

• Education and Experience: High School Diploma or GED; project coordination experience preferred; EPIC EHR experience preferred; BLS certification required.

Responsibilities

• Establishes a dental home for patients without one.
• Collaborates with medical staff to facilitate warm handoffs for dental care.
• Assists dentists with oral health assessments and during Tele-dentistry appointments.
• Re-establishes patients in MCC dental system.
• Manages scheduling, referrals, and patient communication to clinics for urgent care.
• Produces daily reports on fluoride applications and related items.

Overview

Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. As a Federally Qualified Health Center (FQHC), we provide vital health services to almost 40,000 individuals annually in Marin County. The Clinics regularly receive national awards from the Health Resources and Services Administrations (HRSA). Under the supervision of the Chief Dental Officer, the Dental Care Navigator is an advocate for patients and assists dental patients with a variety of questions and concerns in determining treatment needs. These duties include helping patients compile the right documents and communicating with Dental professionals to determine the right place for patients to receive care.

Responsibilities

• Establishes a dental home for patients without one.
• Collaborates with the medical department staff to facilitate a warm handoff for dental care.
• Assists the dentists with oral health assessments via tele-dentistry.
• Assists during Tele-dentistry appointments as needed.
• Assists dentist with dental exam/fluoride application for infants/toddlers without dental home during patient well check visit.
• Re-establishes patients in our dental system.
• Collaborates with the dentists during school screening sessions, assisting the dentists, making appointments, and referring patients to the clinics for urgent care.
• Produces daily reports on potential fluoride applications, etc.
• Proactive management of appointment schedule-- call patients with appointments later in the day as slots become available due to cancellations and no-shows.
• Reminds patients to bring all medications to visit.

Qualifications

Education and Experience:

• High School Diploma or Equivalent (GED) required.
• Project coordination experience preferred.
• Experience with EPIC Electronic Health Records preferred.
• BLS certification required.

Required Skills and Abilities:

• Adheres to confidentiality laws regarding health information protections.
• Ability to speak, read, and writes in English and Spanish required.
• Ability to work independently and exercise good judgment with appropriate supervision.
• Ability to manage and prioritize communications from multiple staff, organizations, patients, etc., at the same time.
• Ability to proofread, correct and review forms and data for scanning and/or entry.
• Basic knowledge of Microsoft Office software required: Outlook, Word, and Excel.
• Ability to work independently and work positively as a collaborative team member.
• Ability to represent clinics professionally.
• Ability to meet deadlines and manage changing priorities.
• Excellent oral and written communications skills.
• Ability to wort at any of the MCC clinics, as needed.

Physical Requirements and Working Conditions:

• Fulfill immunization and fit for duty regulatory requirements.
• Prolonged periods of sitting at a desk and working on a computer.
• Use of mouse and keyboard.
• Moderate to loud noise and intermittent interruptions.
• Must be able to lift to 15 pounds at times.
