Job summary

Genesee Health System in Flint, MI is seeking a qualified professional to join our clinical team in the Assisted Outpatient Treatment (AOT) program under the guidance of the Program Supervisor. The role focuses on coordinating supports and services for adults with serious mental illness or developmental disabilities, aligning with individual plans and IPOS.

Responsibilities

• Under the direction of the Program Supervisor, provides professional services to participants in the county’s Assisted Outpatient Treatment (AOT) program and coordinates supports and services per IPOS.
• Assists individuals and their networks to identify, obtain, and coordinate paid/unpaid supports to enhance independence and integration.

General Statement:

Under the direction of the Program Supervisor, functions as a team member providing professional services to participants in the county’s Assisted Outpatient Treatment (AOT) Program in accordance with the person-centered plans of person served, and coordinate the provision of supports and services for adults with serious and persistent mental illness and/or developmental disabilities who need assistance in accessing, coordinating and monitoring resources and services. Assists the individual and their support network to identify, select, obtain and coordinate both paid and unpaid or natural supports to enhance the person's independence, integration, and productivity consistent with the person's capabilities and preferences as outlined in their Individualized Plan of Services (IPOS); performs related work as required.

Educational Requirements

Bachelor’s degree in Human Services (Or a related field such as Child Development/Family Relations, Occupational Therapy, Community Mental Health, Chemical Dependence, Physical Therapy, Counseling, Criminal Justice, Recreational Therapy, Rehabilitation Counseling, Social Work, Psychology, Education, or Sociology).

Additional Requirements:

Must have a valid Michigan Driver’s License, current automobile insurance, and the use of a personal vehicle during work hours. Must be willing to transport persons served using personal or agency vehicles. Must be willing to obtain and maintain MCBAP credentials appropriate to level of licensure and level of experience.

Preferences:

LBSW or LMSW AND two (2) years of clinical experience with adults with severe mental illness or developmental disabilities.

Essential Physical Demands:

• Vision – Position requires the ability to visually assess details at both near and far distances, as well as to perceive depth and spatial relationships with or without reasonable accommodations.
• Hearing – Position requires the ability to receive and respond to verbal communication with or without reasonable accommodations.
• Lifting, Standing, and Walking With or Without Reasonable Accommodations:
  • Position requires the ability to lift up to 35lbs on an occasional basis
  • Position requires the ability to regularly stand for extended periods of time
  • Position requires the ability to walk at a minimum of 35% up to a maximum of 65%
• Position will require the ability to occasionally stoop, bend, or reach to perform work tasks with or without reasonable accommodation
