Job summary

Sprenger Health Care is seeking a licensed Practical Nurse or Registered Nurse with at least 2 years of long-term care experience to join our team in Mariners Watch, SC. You will direct, evaluate and supervise resident care, develop care plans and participate in care conferences to ensure high-quality service.

You will coordinate admissions and transfers with the Assisted Living Manager and collaborate with a multidisciplinary team to support residents and families.

Qualifications

• Must be licensed LPN or RN and have 2 years of long-term care experience.
• Direct, evaluate and supervise resident care; participate in care conferences.
• Coordinate admissions and transfers with Assisted Living Manager.

Responsibilities

• Direct, evaluate, and supervise all resident care and initiate corrective action as necessary.
• Assess resident care needs and assist in development of individualized service plans.
• Evaluate resident care considering resident needs, family involvement and physician orders.
• Participate in resident care conferences as needed.
• Coordinate admissions and transfers with Assisted Living Manager.
