Job summary

The Pavilion at Stow is seeking a Licensed Practical Nurse (LPN) for a full-time on-site role in Stow, OH. You will provide direct nursing care, supervise CNAs, administer medications, and support holistic resident well-being in alignment with regulatory standards.

Qualifications include an LPN license, CPR certification, and preferably long-term care experience. IV certification is a plus, with a strong emphasis on teamwork and compassionate care.

Qualifications

• Current unrestricted LPN license in practicing state.
• Current CPR certification.
• IV certification, if applicable.

Responsibilities

• Direct nursing care to residents and supervise CNAs per regulations and facility policies.
• Follow nursing model to promote holistic resident care.
• Ensure proper compliance with policies and procedures by nursing staff.
• Assist in admissions, discharges, and transfers as required.
• Assess changes in residents’ status and communicate with physician and family.
• Transcribe orders and administer medications as prescribed.
• Perform wound care and document changes.
• Schedule follow-up appointments and transportation as needed.
• Provide nursing leadership to the unit staff.

Position Summary

The Licensed Practical Nurse (LPN) may provide direct nursing care to the residents and supervises the day-to-day nursing activities performed by certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures.

Key Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.

• Follows the nursing model to promote holistic care for the residents.

• Ensures that policies and procedures are complied with by nursing personnel assigned.

• Participates in the admission, discharge and transfers of residents as required.

• Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.

• Transcribes physician orders to medical record and carries out orders as written.

• Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated.

• Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly as indicated.

• Schedules follow up appointments for residents and transportation needs as indicated.

• Provides nursing leadership to nursing personnel assigned to the unit.

• Other duties as assigned.

Qualifications

• Prior experience in long-term care preferred but not required.

• A Nursing degree from an accredited college or university.

• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.

• Current CPR certification.

• IV certification, if applicable
