Trilogy Health Services in Kokomo, IN is seeking an Assisted Living Coordinator to oversee health concerns of residents, coordinate assessments, medication management, and daily living needs.
You will collaborate with the DON/ADON, ensure pre-admission screens, lead service plan development with residents and families, and support caregivers. LPN license and CPR certification required; leadership experience preferred.
