Job summary

Citizens Options Unlimited in East Shoreham, NY seeks a supervisor to lead direct support professionals in implementing person-centered plans and monitoring progress to ensure quality of care for residents. You will assist the ICF Supervisor, supervise daily data checks, coordinate trainings, and ensure safety and compliance while supporting residents with daily living activities.

Responsibilities

Supervises direct support professionals in implementing the person-centered plan and reviews progress.
Assists and orients new DSP staff, ensuring training on program plans and safety requirements.
Performs daily checks and monitors DSP data (goals, behavior plans, charts, mealtime data).
Supports scheduling and ensures community outings and medical appointments are completed.
Provides direct support for residents' daily living activities and safety precautions as per plans.
Conducts in-service training and participates in meetings to evaluate program effectiveness.
