Job summary

Pathways to Community is seeking an Assistant Site Supervisor in Richfield, MN. This role supports individuals with daily needs, leads scheduling, accompanies clients to medical appointments, administers medications, and ensures safety. Strong leadership, reliable transportation, and DHS background check are required.

Full-time position with 34 hours/week and a compensation of $23 per hour. Must be available to cover shifts and respond by phone Sundays through Wednesdays.

Qualifications

• Strong English communication skills
• Valid driver’s license and daily access to a reliable vehicle
• Ability to cover shifts beyond own as necessary
• Ability to participate in water activities (pool, waterslides, hot tub, etc.)
• Pass a DHS and nationwide background check
• Experience working with individuals with developmental disabilities is required

Responsibilities

• Support individuals with daily needs including personal cares
• Schedule and plan meals and activities
• Schedule and accompany clients to medical appointments
• Administer medications
• Cook and handle housework
• Ensure house safety and provide constant supervision
• Transport to appointments and community events
• Coach and mentor staff
• Maintain documentation and logs in compliance with policies and regulations

Assistant Site Supervisor Schedule:

Direct Care: Wednesday 4:00pm-9:00pm, Thursday, Friday 2:00pm-10:00pm, Sunday 9:00am-10:00pm

Admin Time: 2-6 hours per week

(34 hours per week)

Compensation: $23 per hour

*This position requires you to be accessible by phone Sunday 8:00am–Wednesday 10:00pm

*You will also need to be available to cover any shifts from Sunday 8:00am–Wednesday 10:00pm as needed.

In this role you will be making a difference by:

• Supporting individuals with daily needs including personal cares
• Scheduling and planning meals and activities
• Scheduling and accompanying clients to medical appointments
• Administering medications
• Cooking and housework
• Ensuring house safety
• Provide constant supervision
• Transportation to appointments and community events
• Coaching and mentoring staff
• Maintaining proper documentation and logs of all consumer-based activities, medications, and other items in compliance with PTC and other guidelines, policies, and regulations.

To be considered for this role you must:

• Carry a valid driver’s license, proof of insurance, and have daily access to a reliable vehicle
• Hold an acceptable driving record
• Possess strong English communication skills
• Have the ability to cover shifts beyond you own as necessary
• Be able to participate in water activities (pool, waterslides, hot tub, etc.)
• Be able to pass a DHS and nationwide criminal background check
• Have previous experience working with individuals with developmental disabilities is required.
