The Brooklyn Hospital Center is seeking an Assistant Patient Care Coordinator to manage the assigned Nursing Unit in collaboration with physician leadership. The role includes shift assignments, admissions, transfers and discharges, and acting as a clinical resource to the unit.
You will provide professional nursing care, assess and document care using the nursing process, and support the team while upholding the hospital's standards and mission. NY RN license required.
The Assistant Patient Care Coordinator is responsible for the operational management of the assigned Nursing Unit in collaboration with physician leadership, participates in the strategic planning for the department. Has accountability for nursing services on the assigned shift. Reports to the Nurse Manager and works with related departments on ensuring that patient care issues are addressed. Makes shift assignments and facilitates admissions, transfers and discharges. Acts as a clinical resource for the unit. Provides professional nursing care to all people of all ages and their familes. Utilizes knowledge of nursing process to assess, plan, intervene, evaluate and document care. Formulates nursing diagnosis for treating actual or potential health problems, teaching, counseling and providing care. Delegates appropriate tasks to assistive nursing personnel. Displays professional practice as set forth in The Brooklyn Hospital's standard of care and the institution's mission and goals.
