Job summary

Health First is seeking a Clinical Supervisor for Home Care in Florida to lead the clinical team, ensure Quality/No Harm, and Stewardship. You will help develop and implement policies for the Home Care department and participate in staff interviews, scheduling, and performance appraisals.

POSITION SUMMARY

To be fully engaged in providing Quality/No Harm, Customer Experience, and Stewardship by managing the Clinical Supervisor team. Additional functions include participation in the development and implementation of policies and procedures for the Home Care department. Interview and selection of personnel, scheduling and assignment of personnel, input into employee performance appraisals, quality improvement, and meeting financial targets. Serving as clinical and professional role model to staff and contributing to their professional growth and development as well as fostering connections with customers both internal and external.

PRIMARY ACCOUNTABILITES

• 1. Promotes associate accountability and competency through leadership and management practice. Encourages associate growth and development through mentorship and goal setting.
• 2. Assists in providing consistent on-boarding experience for all new clinical staff.
• 3. Provides input for annual performance appraisals and competency checklists for personnel on an annual basis and as necessary in a timely manner.
• 4. Interviews and participates in the selection of new clinicians.
• 5. Assists with implementation of utilization-based performance improvement activities resulting in improved operations and patient care outcomes.
• 6. Identifies opportunities for quality improvement activities in the department.
• 7. Works with individual nursing supervisors and their clinical teams on opportunities to increase Patient Engagement scores, along with HHVBP incentives.
• 8. Ensures compliance with the Joint Commission and other organizational and regulatory agency standards with quality indicators and core measures with on-going education.
• 9. Monitors and ensures compliance with staffing guidelines, utilization and productivity to meet financial goals.
• 10. Communicates staff, equipment needs and concerns to Manager/Director as appropriate.

Work Experience

MINIMUM QUALIFICATIONS

• Education: BSN
• Work Experience:
  • Three (3) years Home Health nursing experience.
  • Two (2) years acute care experience as a RN.
• Licensure:
  • Current RN Licensure in the State of Florida or endorsement.
  • Valid Driver's License and clean Motor Vehicle Record and maintained.
• Certification: American Heart Association (AHA) Basic Life Support (BLS) Healthcare Provider Completion Card prior to start date and maintained.
• Background Check: Must satisfy Florida of Agency for Health Care Administration (AHCA) Level 2 background screening.

PREFERRED QUALIFICATIONS

• Work Experience: Home Health leadership experience.

PHYSICAL REQUIREMENTS

• Majority of time involves sitting or standing; occasional walking, bending, and stooping.
• Long periods of computer time or at workstation.
• Lightwork that may include lifting or moving objects up to 20 pounds with or without assistance.
• May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
• Communicating with others to exchange information.
• Visual Acuity and hand-eye coordination to preform tasks.
• Workspace may vary from open to confined.
• May require travel to various facilities within beyond county perimeter may require use to personal vehicle.
