Position Type: Per Diem

Duties

• Provide nursing care in accordance with the patient’s plan of care, including comprehensive health and psychosocial evaluation, monitoring of the patient’s condition, health promotion, prevention coordination of services, teaching and training activities, and direct nursing care.

Responsibilities

• Coordinate total patient care by conducting comprehensive health and psychosocial evaluation, monitoring the patient’s condition, promoting sound preventive practices, coordinating services, and teaching and training activities.
• Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis.
• Perform admission, transfer, re‑certification, resumption of care, and discharge OASIS for home care patients.
• Prepare and present patient records to the Clinical Record Review Committee as indicated.
• Consult with the attending physician regarding alterations of patient care plans; check with the appropriate supervisor and make changes when appropriate.
• Coordinate patient services and submit a daily tally of patient‑care visits.
• Participate in case conferences, discuss problems concerning patients with the supervisor, and propose solutions.
• Discuss with the supervisor the need for involvement of other health‑team members such as the Home Health Aide, Physical Therapist, Speech Therapist, Occupational Therapist, or Medical Social Worker.
• Cooperate with other agencies to provide continuity of care and implement a comprehensive care plan.
• Participate in staff development meetings and attend in‑service education, workshops, conferences, and professional organizations to improve nursing care skills.
• Develop and periodically revise the physician’s Plan of Treatment and process change orders as needed.
• Submit clinical notes within 48 hours, progress notes, and other clinical record forms outlining services rendered.
• Participate in the patient’s discharge planning process.
• Maintain an up‑to‑date knowledge of current drug therapy.
• Adhere to federal, state, and accreditation requirements, including Medicare and Medicaid regulations.
• May be requested, by the Clinical Manager, to fill in for other nurses on vacation or sick leave.
• Conduct initial and ongoing comprehensive assessments of patient needs, including OASIS assessments at appropriate intervals.
• Obtain a medical history, conduct a physical examination with vital signs, assess mental status, appetite, and diet, and evaluate the patient’s environment to determine needed support and availability of family assistance.
• Explain nursing and agency services to patients and families as part of care planning.
• Develop and implement the nursing care plan.
• Provide skilled nursing care as outlined in the nursing care plan, including preventive and rehabilitative procedures ordered by the physician.
• Observe signs and symptoms, report to the physician reactions to treatments, and document changes in the patient’s physical or emotional condition.
• Teach, supervise, and counsel patients and caregivers regarding nursing care needs and related issues.
• Supervise and evaluate the care provided by the Home Health Aide at least once every 14 days; submit written evaluations to the appropriate department.
• Participate in conferences with the Home Health Aide supervisor concerning performance.
• Chart services rendered, record changes in patient condition, revise nursing care plans, evaluate patient progress, and close charts for discharged patients.
• Obtain orders for home health aide service and submit referrals to appropriate personnel.
• Provide guidance and supervise the LPN, and supervise the LPN once monthly.
• Participate in educational experiences for student nurses and in the planning, operation, and evaluation of the nursing service.
• Prepare the care plan for the Home Health Aide.
• Use equipment such as thermometer, BP cuff, glucometer, penlight, and hand‑washing materials for patient assessment.

Job Conditions

• Must have a driver’s license and be willing and able to drive to patients’ residences.
• Must be able to access patients’ homes, which may not be routinely wheelchair accessible; hearing, eyesight, and physical dexterity must be sufficient for physical assessment and demonstration of patient care.
• Physical activities include walking, sitting, stooping, standing, and lifting patients.
• Must communicate verbally and in writing in English; frequent telephone communication is required.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed as a Registered Nurse in the state of Florida.

• Knowledge of Medicare and Medicaid guidelines.
• Working knowledge of home health care principles, techniques, and required documentation.
• Skillful in organization, time management, and management processes.
• Ability to contribute to the quality of care through constructive communication with nursing managers and staff.
• Completion of a criminal background check.
• Current CPR certification; online certification is not accepted.
