Qualifications

• Licensed OTR, RPT, SLP, PTA or COTA required for SNF experience.
• Strong interpersonal and written communication skills.
• Ability to manage relationships with referral sources and educate clinicians.

Responsibilities

• Split role: 20% management training, 80% patient care responsibilities.
• Build and strengthen relationships with outpatient referral sources by face-to-face meetings.
• Educate physicians, office managers, and referral coordinators about the facility's services.
