Care Plus Homehealth Care is hiring a full time Assistant Director of Nursing (ADON) to join our team. This dual-role position combines both management responsibilities and direct patient care. Our home health services support residents in senior living communities across Carmel, Indianapolis, and Zionsville, as well as individuals in their homes throughout Boone, Hamilton, Madison, and Marion counties. Certain management duties will require in-person attendance at our main office located in Anderson, Indiana.

ADON will provide direct care and case management to patients in accordance with the State Nurse Practice Act, Agency policy, and accepted professional standards of practice. The ADON supports the DON by sharing responsibility for the delivery of patient care services through coordination, implementation, and supervision of the patients POC and supervision of staff delivering care. Performs essential DON duties in the absence of the DON.

ADON responsibilities include, but are not limited to:

  1. QA documentation
  2. Assist with QAPI
  3. Creating patient education handouts
  4. Completing Patient Risk Identification Forms for falls, infection control, etc.
  5. Intake Referral Process
  6. Required On-Call weekday and weekend rotation
  7. Participates in Community Education (i.e. Blood Pressure Clinic, Resource Tables, Health Fair)
  8. Complete additional tasks assigned by the DON

General responsibilities include:

  1. Performs initial and ongoing patient assessments, including the OASIS, 60-day recertifications and Home Health Aide 14-day Supervisory Visits.
  2. Collaborates with other health care professionals, patients and families developing a comprehensive and coordinated plan for care.
  3. Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing.
  4. Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice.
  5. Practices nursing with respect for individual, cultural, and spiritual differences.
  6. Communicates effectively and in accordance with Agency requirements.
  7. Takes appropriate actions independently and responsively in home care situations to improve the quality of patient care.
  8. Promotes personal safety and a safe environment for patients and coworkers.
  9. Supervises home health care team members.
  10. Performs other duties as assigned.
  11. Experience with blood draws, catheters, IVs, ostomies, PICC lines, and wound care dressings.

Compensation:

  • Full-Time Salaried
  • $86,000- $93,000 per year
