Ebenezer’s Meadow Woods Assisted Living in Bloomington, MN is hiring an Assistant DHS (Director of Health Services) RN. The role ensures safety and health needs of residents while providing staff leadership in a residential setting near East Bloomington.

The position includes 80 hours per two weeks, day shift, and no weekends. The RN will monitor health status, perform regular assessments, coordinate services, maintain medical records, and support families and staff.

Qualifications

• RN license in Minnesota required.
• Associate degree in nursing required.
• 1–3 years of experience preferred.
• BLS certification required.

Responsabilités

• Maintain current assessment of health status.
• Perform assessments per Minnesota regulations and Nurse Practice Act.
• Collaborate with client and family to ensure services and placement.
• Maintain and update service agreements with new tenants/residents.
• Develop and implement emergency health plans, including POLST.

• Coordinate client health services.
• Facilitate coordination with internal and external providers.
• Ensure physician orders are signed and implemented.
• Assist clients with service delivery and funding.

Job Overview

Ebenezer is hiring for an Assistant DHS (Director of Health Services) RN. This nursing position is responsible for ensuring that the safety and health needs of the assisted living clients are met while providing staff leadership duties at our Meadow Woods Assisted Living is located in Bloomington, MN

This positions includes:

• 80 hours per two weeks


• Day shift (Monday - Friday)


• No weekends

As part of the Martin Luther Campus Senior Community, Meadow Woods Assisted Living is a residential setting by woodlands, wetlands, mall of america, and the Minnesota River Bluffs. Our 113 unit assisted, independent and memory care buildings are located in East Bloomington, MN near public transportation. Our staff encourages participation in the many regularly scheduled activities and lifelong learning programs. Responsibilities

• Maintains current assessment and documentation of client’s health status.


• Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensures RN Assessment is completed upon admission, within 14-days of admission, and at least every 90 days and when/if there is a change in health condition.


• Works with client and/or responsible party through collaboration, education and timely communication to ensure appropriate service and placement.


• Assures that clients have up-to-date service agreements. Implements service agreements with new tenant/residents. Reviews and updates service agreements at least once per year and more frequently when there is a change in service needs.


• Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST.


• Maintains clinical charts and progress notes using computerized documentation system.


• Communicates changes in client’s conditions to medical and other relevant individuals.


• Coordinates client health services.


• Facilitates coordination of services with internal and external providers.


• Assures that all physician orders are signed and implemented per the applicable Minnesota Assisted Living regulations.


• Works with client and/or representative(s) to assure delivery of appropriate services and funding.


• Participates in Care Conferences as needed or requested by the DHS or Executive Director

Required Qualifications

• Vocational School /Technical Training or


• Associates Degree in Nursing


• One to Three Years Exerpience


• MN Registered Nurse (RN) License


• Basic Life Support (American Heart Assoc or Red Cross)

Preferred Qualifications

• Bachelor of Science Nursing


• Three to Five Years Experience


• Prior supervisory experience


• Prior experience in home care or assisted living environment
