Ebenezer Meadow Woods Assisted Living in Bloomington, MN seeks an Assistant DHS (Director of Health Services) RN to lead nursing operations, ensure resident health needs are met, and provide staff leadership within a 113-unit community managed by Ebenezer.

The role emphasizes compliance with Minnesota regulations, regular health assessments, and coordination of services with families and providers; day shift, 80 hours per two weeks, Monday–Friday, no weekends.

Qualifications

• RN licensure in Minnesota, active.
• Associates Degree in Nursing.
• 1–3 years of experience in health services
• Basic Life Support certification

Responsibilities

• Maintains current assessment and documentation of client’s health status.
• Performs initial and on-going assessments per Minnesota regulations and Nurse Practice Act.
• Collaborates with client/party through education and timely communication to ensure appropriate service and placement.
• Ensures up-to-date service agreements and reviews them annually or when needs change.
• Develops emergency plans including POLST and maintains charts using computerized systems.
• Communicates changes in client conditions to medical staff and coordinates services.
• Coordinates and coordinates with internal/external providers; participates in care conferences as needed.

Job Overview

Ebenezer is hiring for an Assistant DHS (Director of Health Services) RN. This nursing position is responsible for ensuring that the safety and health needs of the assisted living clients are met while providing staff leadership duties at our Meadow Woods Assisted Living is located in Bloomington, MN

This Positions Includes

• 80 hours per two weeks
• Day shift (Monday - Friday)
• No weekends

As part of the Martin Luther Campus Senior Community, Meadow Woods Assisted Living is a residential setting by woodlands, wetlands, mall of america, and the Minnesota River Bluffs. Our 113 unit assisted, independent and memory care buildings are located in East Bloomington, MN near public transportation. Our staff encourages participation in the many regularly scheduled activities and lifelong learning programs. Responsibilities

• Maintains current assessment and documentation of client’s health status.
• Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensures RN Assessment is completed upon admission, within 14-days of admission, and at least every 90 days and when/if there is a change in health condition.
• Works with client and/or responsible party through collaboration, education and timely communication to ensure appropriate service and placement.
• Assures that clients have up-to-date service agreements. Implements service agreements with new tenant/residents. Reviews and updates service agreements at least once per year and more frequently when there is a change in service needs.
• Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST.
• Maintains clinical charts and progress notes using computerized documentation system.
• Communicates changes in client’s conditions to medical and other relevant individuals.
• Coordinates client health services.
• Facilitates coordination of services with internal and external providers.
• Assures that all physician orders are signed and implemented per the applicable Minnesota Assisted Living regulations.
• Works with client and/or representative(s) to assure delivery of appropriate services and funding.
• Participates in Care Conferences as needed or requested by the DHS or Executive Director

Required Qualifications

• Vocational School /Technical Training or
• Associates Degree in Nursing
• One to Three Years Exerpience
• MN Registered Nurse (RN) License
• Basic Life Support (American Heart Assoc or Red Cross)

Preferred Qualifications

• Bachelor of Science Nursing
• Three to Five Years Experience
• Prior supervisory experience
• Prior experience in home care or assisted living environment
