Job summary

Triangle Springs Hospital is seeking an Assessment Specialist, RN to join our team in Raleigh. This full-time role supports coordinating care, completing assessments, and facilitating admissions with a focus on trauma-informed, patient-centered care.

The position requires an RN license, CPR certification, and CPI training within 30 days. Experience in psychiatric health care and knowledge of EMTALA are highly valued.

Qualifications

  • Nursing degree and state licensure.
  • Experience in psychiatric health care preferred (chemical dependency, dual diagnosis).
  • CPR certification required.
  • CPI training within 30 days of hire.
  • Unencumbered RN license in good standing.

Responsibilities

  • Facilitates response to requests for services, including initial response, assessment, referral, level of care designation, admission, and follow-up.
  • Conducts initial evaluation, documents symptoms, functioning, and needs, and informs MD for LOC determination.
  • Demonstrates excellent customer service to patients, physicians, visitors, and coworkers.
  • Maintains EMTALA paperwork knowledge.
  • Handles preadmission and admission paperwork and insurer precertification.
  • Other duties as assigned by leadership.

Assessment Specialist, RN

Schedule: Full-time, 7a-7:30p

Responsibilities
  • Facilitates response to a request for services, including the initial response, the assessment, and referral process, the designation of the appropriate level of care, admission into services, and appropriate follow‑up activity.
  • Conducts initial evaluation, initiates the assessment, documents patient symptomology, functioning, and needs, and provides information to the MD for LOC determination.
  • Demonstrates excellent customer service skills in assisting patients, physicians, visitors, and coworkers in a prompt and courteous manner.
  • Possesses clear knowledge of Emergency Detention paperwork and processes and EMTALA.
  • Maintains all preadmission and admission paperwork with accuracy and completes all precertification with insurance companies.
  • Other responsibilities as assigned by leadership.
Qualifications
  • Education: Graduate from an accredited professional nursing program or hold relevant state licensure.
  • Experience: Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, and psychiatric patients preferred; experience in patient assessments, family motivations, treatment planning, and communication with external review organizations.
  • License: Current unencumbered license to practice by the State Board of Nursing.
  • Additional Requirements: CPR certification and Crisis Prevention Training (CPI) within thirty days.
