Job summary

Turning Point Community Programs seeks a qualified clinician to provide psychosocial and recovery-oriented services for adults. You will conduct initial assessments, engage clients and families, and support transitions to primary therapy.

The role requires a Master’s in Social Work or related field, 4+ years of mental health experience, and BBS registration. Monday–Friday schedule with on-site duties in Rancho Cordova, CA.

Qualifications

• Master’s degree in Social Work or related field is required.
• Minimum of 4 years’ experience providing mental health services.

Responsibilities

• Complete intake Core Assessments (CANS, CHQ, MSE) in field or office.
• Develop and link to Individual Client Plans with treatment team.
• Provide holistic, recovery-oriented services and crisis intervention.
• Coordinate with prior providers and collateral resources during transitions.
• Facilitate groups and individual/family therapy.
• Ensure timely completion of paperwork and documentation.

Under the general supervision of the Team Leader or Program Director, this at-will direct service position is responsible for providing psychosocial, rehabilitation, and recovery-oriented services for adults. The position is responsible for ensuring that clients are introduced into the Program in a comfortable, thorough, and timely manner. Completes the initial assessment of clients to confirm eligibility under the target population; keeps clients and families engaged in services until assignment to a primary therapist is successful.

Essential Duties And Responsibilities - (illustrative Only)

• Responsible for completing the intake Core Assessment, including collateral assessments (CANS, CHQ, MSE) in the field or in the office.
• Responsible for supporting the development of the Individual Client Plan in collaboration with the member and the treatment team through linkage and handoff at CFT.
• Identifies strengths and challenges of the child and family system.
• Screens for barriers to treatment.
• Identifies the child and family’s natural supports.
• Interacts productively with the treatment team to provide holistic assessment and engagement services that are member-driven and recovery-oriented.
• Completes all required paperwork as assigned promptly.
• Provides prompt intervention in the event of a crisis, both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis.
• Provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program.
• Initiates contact with previous provider, Primary Care Physician, Psychiatric Hospital, and/or collateral resources, to obtain assessment information and provides case management during transition as necessary.
• Ensures that clients discharging from psychiatric hospitals are linked to the agency Psychiatrist within 30 days. Ensures clients transferring to services with medication already in place are linked to the agency psychiatrist.
• Develops initial safety plan and monitors safety planning in high-risk cases during assessment period.
• Completes Service Requests and links clients and families to community resources as appropriate.
• Facilitates groups and provides individual and family therapy.
• Supports Family Advocate with engagement of new referrals to the program.

Qualifications

MINIMUM:

A typical way of obtaining the knowledge, skills, and abilities outlined above is through graduation from an accredited college or university with a master’s degree in Social Work or related field; a minimum of 4 years’ experience as a provider of mental health services, and valid registration with the Board of Behavioral Sciences (BBS).

LICENSES; CERTIFICATES; SPECIAL REQUIREMENTS

• California driver’s license & current vehicle insurance/registration
• Reliable means of transportation capable of passing vehicle safety inspection if more than five years old, excluding all modes of two-wheeled transport, inclusive of bicycles, mopeds, and motorcycles.
• Valid registration with the Board of Behavioral Sciences (BBS)

Schedule

• Monday to Friday, 8:00 AM - 4:30 PM

Compensation:

$34.00 - $36.00 an hour.
