Job summary

Carolina Outreach is seeking a full-time Mental Health Therapist to provide in-home, community-based therapy to individuals and families in North Carolina. The role includes service coordination, crisis intervention, and evidence-based individual and family counseling.

You will work with clients, families, and natural supports to implement person-centered plans, coordinate with care teams, and ensure crisis safety. Competitive salary and benefits are provided.

  • Master's degree in counseling, social work or related field is required.
  • Must be licensed in NC as a LCSW/A or LCMHC/A.

Responsibilities

  • Provide individual, family, and/or group therapy to clients based on needs and evidence-based practices.
  • Perform service coordination, crisis assessment/intervention, and symptom assessment/management.
  • Maintain accurate and timely clinical documentation and comply with standards.
  • Collaborate with care teams and participate in supervision as applicable.
  • Develop and implement person-centered plans and crisis/transition plans when needed.

Carolina Outreach is a group of highly trained behavioral health professionals dedicated to promoting the emotional and physical well-being of our community in a safe and respectful environment.We provide a comprehensive and integrative array of behavioral health services and supports for people of all ages with varying levels of need utilizing an equitable and inclusive approach.We focus on helping our clients tap into and develop their inherent strengths with the goal of supporting the highest quality of life possible for all we serve.About the RoleWe are seeking a full-time Mental Health Therapist to provide community-based, in-home individual and family interventions to Mental Health and Substance Use Disorder clients receiving ACT team services. The person in this position will work with clients, families, and natural supports to provide service coordination, crisis assessment and intervention with ACT consumers. Qualifications: This position requires a associate or full mental health license (i.e LCSW/A or LCMHC/A).The person in this position will work with clients, families, and natural supports to provide the following: service coordination; crisis assessment and intervention; symptom assessment and management; individual counseling and psychotherapy; substance abuse treatment; work-related services; activities of daily living services; social, interpersonal relationship, and leisure-time activity services; support services or direct assistance to ensure that individuals obtain the basic necessities of daily life; and education, support, and consultation.Provide mental health assessments and recommend/implement treatment options based on these assessments.Provide clinical guidance to the team about clients’ mental health issues and develop and implement (or assist in the development and implementation of) clients’ person-centered plans.Provide clinical guidance and/or oversight in the development of crisis plans and transition plans when clients move in and out of services.Provide individual, family, or group therapy to clients, families, and natural supports, based on their needs, diagnoses, and best evidence-based practices.Implement therapeutic interventions, crisis interventions, behavioral interventions, psycho-educational activities, and other best evidence-based practice therapies.Work with clients, families, natural supports, and collateral agencies in reviewing, applying, and achieving goals outlined in the person-centered plan.This description outlines the general scope and level of work for this role.
