Horan Hiring Solutions, LLC seeks an experienced Advanced Registered Nurse Practitioner in Washington to join a patient-centered, community-based health organization. You will work within a PCMH framework alongside physicians, ARNPs, PAs, and nursing staff to deliver high-quality care across a diverse patient population.

The role emphasizes advanced clinical assessment, collaborative care, and patient education.

Qualifications

• ARNP program graduate and WA licensure required.
• Prescriptive authority in Washington required.
• Broad primary care clinic experience preferred.
• Strong knowledge of nursing process: assessment, planning, implementation, evaluation.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patients' health goals.
• Maintain timely chart documentation (within 48 hours, 95% of time).
• Provide care coordination through daily huddles within the PCMH model.
• Identify at-risk patient populations and monitor panel health.
