Job summary

Responsibilities

• Perform face-to-face assessment screenings on Hospice patients.
• Prescribe medications and manage dosing based on assessments.
• Develop and adjust hospice treatment plans with the Hospice Medical Director.
• Provide care across Routine, General Inpatient, Respite and Continuous Care.
