Job summary

MultiCare Hospice is seeking an RN Case Manager to join our experienced team. You will coordinate and deliver patient care, guiding progress toward clinical, quality, and cost-effective outcomes under the nursing process.

You will collaborate with nurses and care teams, advocate for diverse patients, and support families. A BSN and WA RN license are required; hospice experience preferred. Shift 5pm–5am on a 7-on/7-off schedule.

Qualifications

  • Bachelor's degree in Nursing or within five years of hire.
  • MSN/PhD/DNP/ARNP advanced degree preferred.
  • Current RN license in Washington State or MLS endorsement.
  • Current BLS certification.
  • 1 year of Hospice experience preferred.

Responsibilities

  • Organize patient care by prioritizing, delegating, delivering and evaluating ongoingly.
  • Deliver nursing care using the nursing process per hospital policies, procedures and protocols.
  • Collaborate with the nursing team to improve quality of care, patient experience and cost reduction.
  • Foster a learning environment for patients, families, and health-care team.
  • Advocate for ethical and holistic care respecting diverse backgrounds.

Position Summary

We’re seeking an RN Case Manager to join our incredible MultiCare Hospice team. Responsibilities
  • You will organize patient care by prioritizing, delegating, delivering and evaluating on an ongoing basis
  • You will utilize the knowledge base of nursing education and experience to deliver nursing care using the nursing process according to hospital policies, procedures and protocols
  • Collaborate with the nursing team to facilitate overall improvement in quality of care, patient experience and cost reduction
  • Foster a learning environment for patients/families, nursing staff and members of the health care team
  • Advocate for ethical and holistic care promoting the autonomy, dignity, and health of patients from diverse ethnic and social backgrounds
Requirements
  • Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service may be considered in lieu of education agreement
  • Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred
  • Current Registered Nurse license in Washington State or Multistate License endorsement (MLS)
  • Current BLS Certification
  • 1 year of experience in Hospice (preferred). other nursing experience will be considered by hiring manager.

FTE: 1.0 , Shift: 5pm-5am , Schedule: 7 on 7 off schedule (Th-Wed)

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $109,262.40 - $194,251.20 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
