The Registered Nurse (RN) is responsible for delivering competent, quality patient care through development, implementation, and evaluation of individual care plans. The RN provides case management and directs patient care following professional standards, agency guidelines, policies, and procedures. He/she acts as a liaison between the agency, patients, and their families.

Duties/Responsibilities
  1. Develop, implement, and evaluate care plans aligned with physician orders, assessing patient needs, condition, and environment, consulting with health team members, updating plans as needed, and documenting following agency procedures.
  2. Deliver competent, quality home care including observation, assessment, nursing diagnoses, counseling, health teaching, and health maintenance/prevention.
  3. Manage assigned caseload and coordinate related services.
  4. Evaluate admissions against set criteria.
  5. Develop, implement, and evaluate individual patient care plans.
  6. Maintain communication with supervisors and review committees as needed.
  7. Act as liaison with patients and families regarding services.
  8. Provide teaching, supervision, and counseling to patients and families about home care.
  9. Schedule workload efficiently.
  10. Participate in rotating weekend, night, and holiday calls if applicable.
  11. Participate in agency evaluation programs.
  12. Coordinate with health team members to ensure effective patient care.
  13. Follow and promote agency policies.
  14. Maintain ongoing professional development through education, conferences, and memberships.
  15. Consult with physicians regarding care plans and patient status.
  16. Participate in staff training and supervise nursing personnel and aides.
  17. Write discharge summaries upon service discontinuation.
  18. Supervise and assign patient care tasks to LPNs and aides, maintaining responsibility for care quality.
  19. Conduct assessments and supervision visits for non-medical support services as assigned.
Physical Requirements
  • Sit, stand, and walk regularly on varied surfaces.
  • Use hands for handling, feeling, and talking/hearing.
  • Occasionally reach overhead or beyond midline.
  • Use hand controls and climb ramps/stairs as needed.
  • Have specific vision capabilities including close, distance, and peripheral vision, depth perception, and focus adjustment.
  • Utilize universal safety precautions.
  • Lift, push, pull, and carry at least 50 lbs.
  • Stoop and bend for up to 2.5 hours per shift.
Education/Qualifications
  • Minimum six months recent home care nursing experience, or hold a public health nursing certificate, or two to three years hospital nursing experience.
  • Current nursing license in the state of employment.
  • Graduate of an accredited nursing school.
  • Satisfactory references and completion of the agency RN qualifying exam.
  • Current CPR certification.
