Community Health and Wellness, LLC is seeking professional Aquatic Therapists to provide services for individuals with disabilities across Texas. Current openings are in San Antonio, TX, offering Flexible Hours and Competitive Rates. Contractors need reliable transportation and must be willing to travel to clients' homes and facilities.

Job Description:

• Provide one-on-one Aquatic Therapy to children and adolescents with various disabilities
• Create tailored goals for clients
• Document each session accurately
• Communicate with team members and service providers
• Submit timely documentation including invoices and progress notes

Essential Duties and Responsibilities:

• Travel to clients' homes as needed
• Transport clients during visits
• Communicate effectively with healthcare professionals, clients, and families
• Use a private laptop or desktop for HIPAA compliance
• Track goals and progress using Microsoft Word, Excel, and Adobe Reader
• Implement care plans through community activities
• Work independently and manage own schedule

Requirements:

• Resume
• CTRS, TRS/TXC, or LMT certification
• CPR/AED/FIRST AID certification (American Red Cross or American Heart Association)
• Basic Water Rescue and/or Lifeguard Training
• Social Security Card
• Driver's License and Auto Insurance (minimum 100/300/100 coverage)
• Professional Liability Insurance (coverage of $1M/$3M)
