Job summary

Griffin Health Brand is seeking an experienced nurse practitioner with RN background to lead wound care at the point of care. The role includes debridement, topical wound care, and coordinating related services to optimise healing outcomes.

The ideal candidate will interpret diagnostic tests, advise on follow-up, and educate patients and other clinicians on current wound management practices within regulatory scope.

Qualifications

• RN with 5 years of RN experience and licensed to practice.
• NP with at least 1 year of experience as an NP.
• Wound care certification preferred.

Responsibilities

• Debridement of wounds by sharp, enzymatic, autolytic or surgical methods as part of patient visit.
• Apply and manage topical wound care and related products.
• Advise and coordinate services (labs, imaging, referrals, home health, VAC, orthotics) to improve healing.
• Interpret diagnostic tests and plan appropriate follow-up.
• Recommend medications or DME to facilitate wound healing within scope.
• Assess diabetic foot ulcer risk per ADA guidelines.
• Educate patients on wound management and systemic therapies.
• Educate PCPs and referring physicians on current wound practices.
• Provide updates to PCP/CM/referring physician on patient condition.
• Serve as resource for decisions, planning, and policies affecting patient health.
• Collaborate with staff to identify needs and help develop plans.
• Additional duties as assigned

Job description

• Debridement of wounds by sharp, enzymatic, autolytic or surgical methods able to be safely completed in the place of patient visit and all associated procedures to complete the debridement.
• Application and management of topical wound care and associated wound management products.
• Direction and recommendation of associated services to improve wound healing outcomes. This includes but is not limited to labs, radiologic tests, vascular tests, referrals, home health care, Vacuum Assisted Closure device, and orthotics.
• Interpretation of diagnostic tests and recommendation of appropriate follow up.
• Recommendations for prescription of required medications and/or Durable Medical Equipment or supplies (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice, license and as per the collaborative agreement with the supervising physician.
• Diabetic Foot Ulcer risk assessment for Members with Diabetes, as per the ADA guidelines
• Education of patients as to the topical and systemic management of their wounds.
• Education of the Primary Care Physician (PCP), CM, or referring Physician on current wound management practices.
• Provision of regular updates to the PCP, CM, or referring Physician on the status of their patient’s systemic conditions associated with wound care.
• Acts as a resource person for decisions, planning, and policies affecting the health of residents.
• Collaborates with staff to identify customer needs and to help develop plans to meet these needs.
• Other duties as assigned

Requirements

Registered nurse (RN) preparation and Nurse Practitioner (NP) license required.

Minimum of 1-year experience as an NP and 5 years’ experience as an RN.

Certification from the Wound Ostomy and Continence Nursing Certification Board preferred.
