Job summary

UCFS Healthcare in Connecticut seeks a skilled APRN to deliver high-quality, patient-centered primary care. Responsibilities include comprehensive exams, preventive care, patient education, and care coordination with physicians and staff.

The role emphasizes adherence to clinical guidelines, timely documentation, and participation in performance improvement. A full-time schedule, Monday–Friday, 40 hours per week, is offered with competitive benefits.

Qualifications

• 1 year of ambulatory care experience preferred.
• Master of Science in Nursing or equivalency required.
• Active CT APRN, CT RN, CT CSP, and DEA licenses.

Responsibilities

• Provide primary health care including preventive services and routine sick care.
• Develop and implement patient management plans and records.
• Perform minor procedures and coordinate care with other providers.
• Counsel patients and families on adherence, growth, and health promotion.
• Maintain accurate medical records and ensure timely chart completion.
• Respond to patient inquiries and document follow-ups promptly.
• Participate in safety, QA, and performance improvement activities.
