Job summary

Physicians Alliance of Connecticut (PACT, LLC) in Hamden, CT seeks an Advanced Practice Registered Nurse (APRN) to provide longitudinal and urgent care in a physician-led practice. The APRN will develop adult/geriatric panels, educate patients on wellness visits, and create comprehensive care plans while coordinating with physicians and nursing staff.

The position requires an APRN with masters or doctorate training and national NP certification (AANP/ANCC), CT RN and APRN licenses, DEA and CT

Qualifications

• Master's or Doctorate prepared APRN.
• Certified nationally as NP via AANP or ANCC.
• Minimum 2 years experience as APRN.
• CT RN & APRN license required; NP certification by approved body.

Responsibilities

• Provide longitudinal and urgent care to patients in an office location.
• Develop adult/geriatric patient panels.
• Educate patients on wellness visits vs. physicals.
• Create care plans and coordinate care with physicians.
