Job summary

Physicians Alliance of Connecticut (PACT, LLC) is seeking an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) to work in Skilled Nursing Facilities in the Hartford area. Travel to contracted facilities and operate autonomously in a pay-for-performance environment.

The role emphasizes high-quality patient care, complex case management, and coordination with facility teams to develop comprehensive care plans. On-site positions in Hartford, CT, full-time are available.

Qualifications

APRN must have master's or doctorate; licensed in CT as RN and APRN with national certification (AANP/ANCC).
PA must have Physician Assistant Certification and CT State License; licensed to practice.
DEA license and CT controlled substances license required.
Experience with elderly populations and EHR systems preferred.

Responsibilities

Provide medical care to patients in short-term rehab and long-term care settings.
Analyze medical history, allergies, and condition to determine care.
Order, interpret, and evaluate diagnostic tests.
Prescribe medications and treatment regimens.
Record findings and plans in the medical record.
Coordinate with facility care planning team on comprehensive plans.
Communicate with patients and families about care and prognosis.
Follow department policies, QA and infection control standards.
