Physicians Alliance of Connecticut (PACT, LLC) seeks an APRN or PA to provide medical care in Skilled Nursing Facilities around Stamford. The role offers a flexible schedule with pay-for-performance potential and travel to contracted facilities in the Stamford area.

The candidate will assess, diagnose and treat complex cases, order tests, prescribe treatments and coordinate with facility care planning for optimal patient outcomes. A strong license, certification and EHR experience are required.

Competenze

• APRN/Masters or Doctorate prepared.
• National NP certification required (AANP or ANCC).
• CT RN and APRN licenses and NP certification required.
• DEA and CT controlled substance license.
• Experience with elderly patient population.
• Experience using EHR systems.

Mansioni

• Provide medical care to complex patients in short-term rehab and long-term care.
• Assess medical history, allergies, and conditions to determine care.
• Order and interpret diagnostics to identify health needs.
• Prescribe medications and treatment regimens as appropriate.
• Document findings and plans in medical records.
• Coordinate with facility care planning team on care plans.
• Communicate admission and discharge needs with patients and families.
• Follow departmental policies, QA objectives, safety standards.

Formazione

APRN: Masters or Doctorate prepared
