Job summary

TCA Health in Hammond, Indiana seeks an Advanced Practice Provider to deliver comprehensive, patient-centered primary care within scope of practice. Responsibilities include history-taking, exams, diagnosis, treatment planning, and care coordination within a collaborative team.

You'll practice under Indiana law, provide preventive care, and participate in telehealth when appropriate. Candidates should have NP/PA credentials, board certification, and DEA/CS registration where required.

Qualifications

  • Current, unrestricted Indiana RN license or qualifying multistate compact RN license.
  • Current Indiana APRN prescriptive-authority license, when required.
  • Master’s or doctoral degree from an accredited nurse practitioner program.
  • Current national board certification in an appropriate primary-care specialty.
  • Current Indiana Controlled Substances Registration and DEA registration, if prescribing controlled substances.
  • Graduation from an accredited physician assistant program.
  • Current NCCPA certification.

Responsibilities

  • Obtain and document comprehensive patient histories and perform physical examinations.
  • Diagnose and manage acute and chronic medical conditions.
  • Develop, implement, and evaluate individualized treatment plans.
  • Order, perform, and interpret appropriate laboratory and diagnostic studies.
  • Prescribe medications and treatments within applicable Indiana authority.
  • Provide preventive care, screenings, immunizations, and chronic disease management.
  • Educate patients regarding diagnoses, medications, treatment plans, and healthy behaviors.
  • Coordinate referrals and collaborate with physicians, nurses, behavioral health professionals, and other members of the care team.
  • Provide telehealth services when clinically appropriate.
  • Maintain timely, accurate, and complete documentation in the electronic health record.
  • Participate in quality-improvement, peer-review, clinical department, and staff meetings.
  • Follow TCA Health policies, clinical guidelines, productivity expectations, and applicable regulatory requirements.

Position Summary

The Advanced Practice Provider provides comprehensive, patient-centered primary care within the provider’s education, training, licensure, and approved scope of practice.
