Ohio’s Hospice in Dayton, Ohio seeks an Advanced Practice Registered Nurse (APRN) to provide expert palliative and hospice care to patients and families. You will assess complex needs, diagnose, develop care plans, and manage symptoms with a collaborative interdisciplinary team to promote comfort and quality of life.

The role emphasizes advanced clinical decision-making, coordination of care, and education for patients and families, with an 8:00 AM–5:00 PM schedule in Dayton.

Qualifications

  • Master of Science in Nursing (MSN) required.
  • Minimum 2 years of APRN clinical experience.
  • Active, unrestricted RN license in Ohio with no board actions.
  • Board certification as an APRN.
  • Active, unrestricted DEA license.
  • Eligibility to maintain malpractice coverage.
  • Current CPR certification.
  • Strong computer skills and ability to accurately document patient care.
  • Valid driver's license with a safe driving record.
  • State-required automobile insurance coverage.
  • Ability to drive during daytime, nighttime, and inclement weather.

Job Summary

Ohio’s Hospice is seeking a compassionate and experienced Advanced Practice Registered Nurse (APRN) to provide expert care to patients living with serious and life-limiting illnesses and their families.

The APRN provides advanced clinical expertise to assess complex patient needs, diagnose conditions, develop and coordinate individualized plans of care, and manage symptoms. This role works closely with patients, families, caregivers, and the interdisciplinary care team to promote comfort, improve functional status, enhance communication, and support the highest possible quality of life.

Key Responsibilities
  • Provide advanced clinical care to patients with serious and life-limiting illnesses.
  • Perform comprehensive patient assessments and synthesize complex clinical information.
  • Diagnose and evaluate patient conditions within the scope of APRN practice.
  • Develop, implement, and coordinate individualized plans of care.
  • Assess and manage pain and other symptoms to promote comfort and quality of life.
  • Monitor patient progress and adjust the plan of care as needed.
  • Collaborate with physicians and members of the interdisciplinary team to ensure coordinated, patient-centered care.
  • Communicate effectively with patients, families, caregivers, and healthcare providers regarding goals of care, treatment options, and changes in condition.
  • Provide education and support to patients and families throughout their hospice and palliative care journey.
  • Accurately and timely document assessments, interventions, and plans of care in the electronic medical record.
  • Serve as a clinical resource and advocate for patients and families.
Qualifications

Required:

  • Master of Science in Nursing (MSN)
  • Minimum of 2 years of clinical experience as an APRN
  • Active, unrestricted Registered Nurse (RN) license in the State of Ohio with no current board actions
  • Board certification as an APRN through a nationally recognized certifying agency
  • Active, unrestricted DEA license
  • Eligibility to maintain adequate malpractice coverage
  • Current CPR certification
  • Strong computer skills and ability to accurately document patient care
  • Valid driver's license with a safe driving record
  • State-required minimum automobile insurance coverage
  • Ability to drive during daytime, nighttime, and inclement weather conditions

Preferred:

  • Certification in Hospice & Palliative Medicine (ACHPN)

Certification Requirement:

  • ACHPN certification is required within two years of employment.
  • Candidates are eligible for certification after completing 500 hours of hospice and/or palliative care experience.
