Job summary

Duly Health and Care is seeking a Physician Assistant or Nurse Practitioner to join our nephrology team in Downers Grove and Aurora. The role blends outpatient clinic visits with dialysis rounding to deliver comprehensive care for patients with chronic kidney disease.

You will assess patients, build care plans with supervising physicians, and coordinate dialysis and related services. A Master’s degree and IL licensure are required, with 1–2 years in nephrology preferred.

Qualifications

• Graduation from an approved school for Physician Assistant or Nurse Practitioner.
• Masters Degree.
• Licensed or ability to obtain licensure in the state of Illinois.

Responsibilities

• Provide high-quality, evidence-based care to nephrology patients in both clinic settings.
• Conduct thorough patient assessments, develop care plans, and manage ongoing treatment with supervising physicians.
• Perform dialysis rounding, including evaluating patients and coordinating care with dialysis center staff.
• Document all patient encounters and care plans accurately in Epic.
• Communicate with patients, families, clinic staff, and interdisciplinary teams.
• Support nephrology providers with patient follow-up, education, and chronic disease management.
• Participate in quality initiatives, care coordination, and process improvement.
