Job summary

Duly Health and Care Bloomingdale is seeking an advanced practice provider to empower health and elevate patient care within a large multi-specialty network. You will practice at the top of your license, collaborate with physicians, and engage with a broad internal referral network across ICC and other Duly providers.

The position offers a base compensation range of $100,000‑$140,000, with potential sign‑on bonus, CME funds, and a 401K match.

Qualifications

• Graduation from an approved school for Nurse Practitioners OR Physician Assistant-Masters Degree.
• Licensed or ability to obtain licensure in the state of Illinois.
• Previous experience (3+ years) in ICC, ER, Urgent Care or similar setting.

Compensation

***The anticipated base compensation range for this position is $100,000-140,000, depending upon the applicant’s qualifications and experience.

In addition to the base compensation, this position may also be eligible for a sign‑on bonus.
