Job summary

The South Bend Clinic is seeking a Nurse Practitioner or Physician Assistant to join our Family Medicine team in Granger, Indiana. The position involves travel to the Chicagoland area for initial training, then practicing in South Bend, IN, delivering patient-centered primary and urgent care, with collaborative physician oversight.

Qualifications

  • Master's degree from an accredited Physician Assistant or Nurse Practitioner program.
  • National cert in applicable specialty (ANCC).
  • Current state licensure as PA or Advanced Practice Nurse and RN.
  • Maintains current CPR (BLS) certification.
  • Active Mid-level Practitioner Controlled Substance License and DEA License may be required based on prescriptive authority.

Responsibilities

  • Perform patient examinations and treatment and record physical exams, analyze data, develop care plans, and treat minor illnesses and injuries per established protocols.
  • Assist with procedures or conduct invasive/non-invasive procedures within scope of practice; order laboratory, x-ray, and other non-invasive tests.
  • Initiate collaboration with supervising physicians and referrals to appropriate health care professionals for problems beyond NP scope.
  • Finish communications to referring provider or supervising physician.
  • Prescribe medications within scope of practice with physician oversight and necessary license.
  • Instruct patients and families on healthcare, prevention, and home care, adjusting to age, psycho-social, and cultural needs.

The South Bend Clinic strives to make the entire Michiana community healthier by providing convenient primary, specialty and urgent care services close to home. Our Family Medicine team in Granger is growing and looking to add a skilled and compassionate provider to join our tight knit and collaborative team, helping to expand our high-quality care and continue serving the needs of our patients with expertise and dedication. We are seeking a Nurse Practitioner/Physician Assistant for this role. New grads are welcome to apply! This position will require consistent travel to Chicagoland area for training the first few months. After training, this position will work in South Bend, Indiana.
