Eventus WholeHealth is seeking an Advanced Practice Clinician (NP/PA) to deliver comprehensive primary care in skilled nursing and senior living communities. You will perform routine rounding, chronic disease management, and coordinate care with interdisciplinary teams and facility partners.

The role emphasizes preventive, value-based care and collaboration with residents, families, and staff to improve outcomes and experiences in long-term care settings.

Qualifications

• Active NP or PA license in the practice state.
• Clinical experience providing patient care.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Strong patient-centered service skills and compassionate communication.
• Ability to collaborate across interdisciplinary teams.

Responsibilities
